Annual plan fee includes:
Two routine preventive visits at 100%—exam, x-rays, and cleaning in the absence of gum disease (prophylaxis)
One emergency visit exam and x-rays at 100% – exam and x-rays (If gum disease is present, two periodontal maintenance cleanings will be covered by the annual plan fee in place of the routine cleanings. Additional periodontal maintenance cleanings will receive a 15% discount.)15% off all other dental procedures:

Annual plan fee may change at the discretion of Tosh Dental Group. Payment for treatment is due in full when services are rendered. It is the sole responsibility of the member to maximize their benefits by arranging all the appropriate appointments within the 12-month membership period. If the appointments are not used, the member will not be entitled to a refund.Exclusions
This savings plan is NOT a dental insurance plan and can only be used at Tosh Dental Group. The plan does not meet all the requirements for coverage of pediatric oral services as required by the Affordable Care Act. Membership is non-transferable and cannot be combined with any other dental insurance coverage or discounts. Discounts do not apply to any treatment prior to joining the plan. Does not apply to any treatment, which in the sole judgment of Tosh Dental Group, lies outside of their scope of care, is covered by medical or automobile insurance, or requires hospitalization.
